SMS Messaging Terms - Digital Rounds
Text message reminders for patients enrolled in Remote Therapeutic Monitoring.
PROGRAM DESCRIPTION
Osleri LLC provides the Digital Rounds platform to healthcare practices that operate Remote Therapeutic Monitoring (RTM) programs. On behalf of the participating practice that enrolled you, Osleri sends a short text message reminding you to complete a brief health check-in survey as part of your RTM care plan. Each message contains a personal link to your survey.
Messages are sent from Osleri because Osleri operates the messaging service for your practice. Your practice remains your healthcare provider and the custodian of your medical record.
HOW TO OPT IN
Patients are enrolled by their practice's care team. During enrollment you complete that practice's Remote Therapeutic Monitoring consent form, which includes a separate, optional text-message consent naming Osleri as the sender - reproduced in full below. You provide your mobile number as part of that consent. We do not add anyone to this program without it.
Text messaging is never started automatically. The text-message consent is a distinct, unchecked item that you must affirmatively select; agreeing to the rest of the RTM consent does not enroll you in text messaging.
Consent is not a condition of care. Agreeing to receive text messages is not required to receive treatment and is not a condition of enrollment in any program or of any purchase. You may decline text messages and still participate fully in your care plan; your care team will use email or phone instead.
PROGRAM DETAILS
Message frequency: Up to one message per scheduled check-in, based on the cadence your care team sets (typically daily, weekly, or monthly). Frequency varies by care plan.
Cost: Message and data rates may apply. Osleri does not charge for these messages; your mobile carrier's standard rates apply.
To opt out: Reply STOP to any message to stop all further text messages. You will receive one confirmation message and then no further texts. You may also ask your care team to change your communication preference at any time. Opting out does not affect your care.

To resubscribe: Reply START at any time to begin receiving reminders again.
Carriers: Carriers are not liable for delayed or undelivered messages.
PRIVACY
Text messages are sent over an unencrypted channel. Our reminders therefore contain no clinical information, no diagnosis, and no test results - only a greeting and a secure personal link. See our Privacy Policy and Terms of Service at www.osleri.com/terms-and-conditions.
ENROLLMENT CONSENT LANGUAGE
Every participating practice's Remote Therapeutic Monitoring consent form includes the following text-message consent. It is a separate item, unchecked by default, and kept apart from the clinical, billing, and privacy agreements - so a patient can join the RTM program without agreeing to text messages.
